Patient Registration
BEFORE YOUR APPOINTMENT:
Please preregister with our office by filling out our secure online medical history update forms or request a virtual appointment if you do not have an in office appointment. The virtual appointment forms will allow the doctors to address your concerns and allow the office to appoint you properly.
In some cases, a simple picture will be needed and can be uploaded to the virtual appointment forms. For example:
- Crown off: Take a picture of the inside of the crown to show if the crown is clean or has tooth structure inside.
- Broken tooth: Take a picture of the broken tooth in and out of the mouth; The doctors can decide on which type of images are needed to further evaluate.
- Trauma to the maxillo-facial region: Take a picture of the traumatized hard and soft tissue.
- Facial swelling, ulcerations, or other abnormal concerns: take a picture of the affected area and list a description of the symptoms you are having.
If you are not sure your case can be treated and diagnosed virtually, please call our office to inquire about how to proceed. A limited exam appointment to take images to aid in diagnosis can be made to address your concerns! Remember, we are here to help in any way we are able!
If you are not technically inclined, that is okay! We will have an assistant to guide you through these forms in our office.

Office Hours
We will schedule your appointment as promptly as possible now that our office has reopened. If you have pain or an emergency situation, every attempt will be made to see you or you may choose to obtain a virtual appointment to receive advice and keep you comfortable until you can be seen for treatment. Due to COVID-19, our office will have reduced hours. We will be seeing emergent patients before we continue non-urgent and elective procedures.
We try our best to stay on schedule to minimize your waiting. Due to the fact Dr. Fetner and Dr. Daverede Hart provide many types of dental services, various circumstances may lengthen the time allocated for a procedure. Emergency cases can also arise and cause delays. We appreciate your understanding and patience.
